  • Overview

    The Health Services Information Specialist reports directly to the Administrative Coordinator and is responsible for performing administrative duties for Health Services.


    • Archive case records upon discharge.
    • Pick up, distribute department mail daily.
    • Maintain confidentiality of residents records.
    • Track patient outcomes for quality assessment.
    • Send alerts to the nurses when yearly physicals need to be completed.
    • Organize and maintain data using departments tracking systems, registries and QI.
    • Review and prepare case records for departmental audits governed by OCFS and/or ACS.
    • Electronically record data for collection, storage, analysis, retrieval, and reporting.
    • Utilize Connections for documentation of ALL medical/mental health information as required.
    • Mission Effectiveness Liaison; attend quarterly meeting with Executive Director, facilitate monthly department meetings and submission of meeting minutes to Executive Assistant.
    • File all medical/mental health documentation daily for RTC, Mother/Child and NSP Bronx programs.
    • Review residents records for timeliness, completeness, accuracy, and appropriateness of data.
    • Prepare charts for new admissions and request paperwork from previous placement from Intake.  
    • Handle all requests/subpoenas for copies that are needed from the case records from outside referrals in accordance with HIPPA regulations, once approved from Chief Privacy Officer
    • Requests records and clinical information from external providers, maintaining compliance as required by the governing bodies.
    • Maintain psychiatric appointments on shared calendar, scheduling med reviews, missed appointments, tracking kept appointments on excel spreadsheet.
