  • Overview

    Limited Secure Placement is part of a groundbreaking initiative to provide services and supports for juvenile justice involved youth in residential placement facilities closer to their families, communities and schools. The main responsibility will be to provide medical care and follow up with direct medical support to individual supported.  Full time morning and evening shifts are available as well as weekend and holiday per diem shifts available.







    Other duties include but, are not limited to the following:


    • Provides general medical care and treatment to individual supported per program, on site.
    • Performs physical examinations and preventative health measures within prescribed guidelines and instructions.
    • Provide health evaluations and testing with follow ups.
    • Review all Pediatric guidelines for care, including immunizations and testing
    • Orders, interprets and evaluates diagnostic test to identify and assess patient’s clinical problems and health care needs.
    • Records physical findings and formulates plan and prognosis based on patient’s condition.
    • Discusses case with collaborating physician, clinical team and other health professional to prepare comprehensive patient care plan.
    • Prescribes or recommends medication or other forms of treatment related to therapeutic needs.
    • May refer patients for consultation or to specialized health resources for treatment, in collaboration for overseeing physician.
    • Conducts medical examination of youth after incidents.
    • Submits health care plan and goals of individual patients for review by physician.
    • On call for all triage, sick, urgent and quick coverage, questions and consult calls for assigned programs.
    • Administer Medication.  
    • Collaborate with LSP staff on medication administration when medical staff is not on site.
    • Train staff.
    • Other duties as assigned.



    • NYS Licensed Nurse Practitioner or Physician’s Assistant
    • Prescribing abilities in NYS with valid malpractice coverage
    • Able to encourage positive attitude toward consumer’s capabilities and limitations
