Overview

 

 

The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program, and the Therapeutic Group Home programs located at our licensed residential facility in Yonkers, NY as well as a Transitional Foster Care program and the Long Term Foster Care Program located in foster homes in the Bronx, Queens and Westchester. In all programs, we provide therapeutic and educational services and other supports that youth need to achieve success through an array of strength-based and targeted interventions as well as link them with family members and/or other sponsors in the United States so they are able to move forward with their lives and thrive. The Nurse will provide direct triage or first aide medical care and medication administration along with direct medical support and coordination of medical appointments to persons supported in the UC program.

Responsibilities

  • Responsible for scheduling, tracking and coordinating all outside medical services for all youth in the program.
  • Maintaining MARS, referrals, labs and follow ups
  • Working closely with staff and person supported to ensure medical well-being of all persons served within a safe and clean environment
  • Ensuring effective medical care, proper documentation, and reporting of all administration of medication to person served.
  • Calculating, monitoring, and analyzing reports regarding medication administration and lab results
  • Monitoring and documenting all occurrences of shift and logging
  • Being on call for emergencies with prompt response time
  • Providing immediate reports to managers of any discrepancies found on the Medication Administration sheets.
  • Notifying management of and follow ups in any emergency medical care for consumers
  • Other duties as assigned

Qualifications

  • LPN with 2 years of clinical/case management or RN NYS nursing License with minimum 2 years of experience
  • Experienced in medication administration, first aid and triage
  • Working knowledge of Microsoft Office (Word, Excel)
  • Able to encourage positive attitude toward consumer’s capabilities and limitations

 

Salary Rage $75,500 - $79,999.92
