Overview

Rising Ground provides caring support and proven paths to positive change, helping children, adults, and families rise above adversity, and each year, Rising Ground is a positive force in the lives of more than 25,000 individuals. We do this through 50 programs in 70 locations throughout New York City and Westchester. Our strong belief and commitment to diversity shows not just through our work with supported persons, but is integral to the building of a strong staff that reflects the communities we serve.

Atlas seeks to address the risks and needs of individuals released on their own recognizance citywide who are at heightened risk of future victimization or justice system involvement. FF-CM is an intensive, family-focused, case management approach that is designed to motivate and link families to resources and services to address referral behaviors and family risk factors. FF-CM is based on the same principles of the evidence-based FFT therapeutic approach: relational-based, strength-based, and respect-based. A key difference, however, is that the FF-CM service is focused on creating linkages to existing community resources to facilitate change.

Interventionists first engage families into the change process and motivate them to connect to services. The second part of the process involves Supporting and Monitoring families as they participate in services. Finally, interventionists help the families to Generalize gains and new skills by locking in these changes and planning for the future. The goal of the implementation of the FF-CM model is to increase the engagement of young adults into therapeutic services, namely Functional Family Therapy.
