The Case Manager will play a critical role in Rising Ground’s Community-Based Services for Survivors of Trauma, Violence and Crime by supporting individuals and families in navigating complex systems such as housing, public assistance, health care, and legal advocacy. We are seeking a passionate, trauma-informed professional who is deeply committed to anti-oppressive practice and social justice. The ideal candidate will not only assist survivors with their immediate needs but will also help empower them to overcome the systemic barriers that contribute to instability and harm. This position will be based out of our Bronx location, with the opportunity to work from our Harlem office and have a remote/hybrid schedule.

 

As part of our team, the Case Manager will work within a restorative justice framework to support survivors in reclaiming their sense of safety, agency, and resilience. You will work collaboratively with a multidisciplinary team to provide advocacy, crisis intervention, and emotional support to those impacted by trauma and violence. This role requires a deep understanding of trauma and its effects, with the goal of offering compassionate, individualized support to help clients stabilize, heal, and achieve long-term well-being.

 

Responsibilities

  • Work directly with survivors to assess their immediate and long-term needs.
  • Advocate for clients in interactions with housing authorities, public assistance programs, health care providers, and the criminal justice system.
  • Support clients through complex applications and paperwork for services such as housing transfers, Medicaid, and public benefits.
  • Provide trauma-informed crisis intervention and emotional support.
  • Connect clients to community resources, including counseling, legal services, and economic empowerment programs.
  • Maintain up-to-date knowledge of relevant services and create individualized care plans for clients.
  • Assist clients in accessing the Office of Victims' Services (OVS) compensation program and process applications as necessary.
  • Maintain a caseload of 20-30 clients, providing consistent, compassionate support.
  • Conduct client intake assessments, develop service plans, and track progress toward stabilization.
  • Coordinate interdisciplinary case management and collaborate with external partners and agencies to ensure comprehensive support.
  • Participate in community outreach to increase access to services for survivors.
  • Engage in continuous professional development related to trauma-informed care, social justice, and anti-oppressive practice.
  • Maintain accurate and confidential records of client interactions and services provided.
  • Complete reports and documentation required by Rising Ground and external agencies.
  • Participate in supervision, staff meetings, and training sessions as required.

Qualifications

  • Bachelor’s degree in social work, human services, or a related field is preferred; however, candidates with significant experience in case management, particularly working with survivors of trauma, violence and crime, will also be considered.
  • Experience working with survivors of trauma, violence, and/or crime, with a strong understanding of trauma-based work.
  • Knowledge of NYC’s public assistance, housing, Medicaid, and employment services.
  • Passion for social justice, anti-oppressive practice, and the ability to work effectively with diverse populations.
  • Strong organizational skills and the ability to manage a caseload in a high-demand environment.
  • Excellent interpersonal communication, writing, and documentation skills.
  • Commitment to person-centered and trauma-informed practice.
  • Ability to be flexible, to work independently and collaboratively within a team.
  • Bilingual in English and Spanish or Haitian Creole preferred.

Work Environment:

Hybrid (In-office and remote), with flexibility to meet clients at various community locations or virtually. This position will be based out of our Bronx location, with the opportunity to work from our Harlem office and have a remote/hybrid schedule.

 

Position Type/Expected Hours of Work: 35 hours a week, Monday through Friday 9am-5pm

 

Travel (if required): May be required depending on client needs.

 

Salary: $58,000
