The Unaccompanied Children Program provides Basic Shelter and Transitional Residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program located in the communities as well as a Transitional Foster Care program located in foster homes in the Bronx, Queens and Westchester. In all programs, we provide therapeutic and educational services and other supports that youth need to achieve success through an array of strength-based and targeted interventions as well as link them with family members and/or other sponsors in the United States, so they can move forward with their lives and thrive. 

What You’ll Do - Upon Admission

Lewis and ORR portal.

• Admit children in the system (within 24 hours of admission)
• Download all trigger reports for each minor.
• (Netsmart/Myevolve) Reflect/enter all children in the system as they arrive to program withing 24-48 hours.
• Ensure to enter children’s movement as needed: intakes, discharges, transfers and hospitalizations.
• Send an email to the Assistant Director of Foster Care when movements are completed.
• Coordinating and reporting of daily census for the program sent to administrative staff, ORR, ICE and Catholic Charities of NY.

 Portal Intake Assessment

• Intake Assessment must be completed and in the portal within 24 hours of admission.
• Print and sign Intake Assessments and upload to portal.
• Take a picture of the youth, upload to the Portal
• Print Trigger reports (Medical and Authorization of Placement) and get signatures.
• Send an email to POH informing me of the completion of the Intake Assessment.
• For additional confidential information obtained during the intake, an email must be sent to the assigned CL, CM, Supervisors, Lead Clinician, Director of Clinical and Case Management, AD of Foster Care, and all Program Directors.
• Within 48 hours of admission, complete the Orientation packet with the children and provide one copy of the signed packet to the Case Manager and one to the child.
• Ensure that the approved list of children’s contacts, distribution list and allergy list are sent to the teams within 24 hours of admission
• Responsible for uploading all documents to the portal account for each minor that arrives withing 24 hours of the minor’s arrival.

Orientation Process:

• Complete Orientation sessions with all new minors within 48 hours of admission to the program. Complete orientation tracker. Sent notifications of completion of orientations.
• Provide Orientation packet to Case Managers to file.
• Ensure complete 90-day orientation sessions with all minors in program and track orientations.
• Discuss with the children’s program expectations, Children’s Rights and Responsibilities, Grievance Letter and Values.
• Introduction to Behavioral Plan and PBIS (Gotchas and Check-in)

Internal and Critical Systems:

• Responsible for entering all SIR’s (Serious Incident Reports) in the internal system.
• Responsible for entering all SIR’s (Serious Incident Reports) into the Critical system within 24 hours of receiving the information.
• Create quarterly report for program tracking and reporting to PEM department on a monthly basis. This includes tracking of all Initial Intake Assessments, Orientations, 90+ day orientations, all SIR’s. 
• Responsible for performing COVID-19 testing for all POH minors in the program.
• Responsible for updating the daily census and sending it to appropriate parties.
• Responsible for other assigned duties as needed.

• Associate’s degree/Bachelor’s degree Preferred in a Human Services

• Bi-lingual (Spanish/English) a must and other languages
• Excellent verbal, written communication and organizational skills
• Proficient in Microsoft (Excel, Word, PowerPoint)

Supervisory Responsibility

This position does not have direct supervisory responsibilities. The Intake Coordinator will work independently with guidance and support from their supervisor and other team members, collaborating with other program teams for proper service delivery.  

Work Environment:

This role requires four in-office days and one remote day per week, with flexibility for after-hours, weekend, and holiday support as needed. The position involves collaboration with multiple teams, fieldwork, and travel to different facilities to assess while maintaining a safe and proper environment for the children.

Position Type/Expected Hours of Work:
This is a full-time position, days and hours of work are Monday through Friday in our Harlem New York office at 317 Lenox Avenue NY. Work hours options are 9:00 am - 5:00 pm. Days are Tuesday to Saturday, Sunday to Thursdays and hours of work are 11-7, 12-8

• Authorized to work in the U.S.
• Ability to work in-person in Harlem New York for at least 4 days or more per week.
• Ability to travel to other Rising Ground sites (as required)
