The Intake Coordinator is primarily responsible for accepting and processing referrals for the Juvenile Justice for Youth and Family Department (JYF programs: NSP, NSD, LSP) and up to two additional departments as needed. The position is on-call and provides screenings for intakes to decide eligibility as well as if the referral can be accepted. As a member of the organizations centralize referral hub, the coordinator will also be responsible for providing resources to both internal and external referral sources.
Maintain formal tracking system to measure agency service referrals performance as it relates to marketing and relationships with referral resources.
Enters referral and agency intakes information in knowledge management systems (MyEvolv, CONNECTIONS, LEWIS, etc.)
Participate in community partnership meetings and workshops.
Assist in events to represent the Access and Referral department.
Work Environment:
This role requires three in-office days and two remote day per week, with flexibility for after-hours, weekend, and holiday support as needed. The position involves collaboration with multiple teams, fieldwork, and travel to different facilities to assess while maintaining a safe and proper environment for the children.
Position Type/Expected Hours of Work:
This is a full-time position with annual salary $52,000.00. The position operates Monday through Friday in our Harlem New York office at 1751Park Avenue NY, NY. Work hours options are 9:00 am - 5:00 pm.
