Next STEPS (Striving toward Engagement and Peaceful Solutions)
With a partnership between Rising Ground, the NYC Dept. Of Probation, and DYCD Office of Neighborhood Safety formerly MAP, Mayors Action Plan. A collaborative network servicing the Patterson Community. Individual and group mentoring by “credible messengers,” adults who have successfully navigated challenges like those experienced by participating youth, including neighborhood of residence, poverty, and/or justice involvement. Will use of the evidence-based Journaling mentoring curriculum with linkages to Rising Ground employment and mental health treatment services and to other appropriate community providers.
Next Steps Mentoring will provide intervention serving high-risk youth aged 16-24 residing in or near the Patterson NYCHA housing in the South Bronx.
The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program, and the Therapeutic Group Home programs located at our licensed residential facility in Yonkers, NY as well as a Transitional Foster Care program and the Long Term Foster CareProgram located in foster homes in the Bronx, Queens and Westchester. In all programs, we provide therapeutic and educational services and other supports that youth need to achieve success through an array of strength-based and targeted interventions as well as link them with family members and/or other sponsors in the United States so they are able to move forward with their lives and thrive. The Educational Coordinator will oversee education services provided to youth supported by the UC program.

Job Summary: The RN provides care management to Individuals with Developmental Disabilities under the OPWDD regulations. Supervision of Staff in regard to delegated nursing tasks such as but not limited to medication administration, medical care, participation in team meetings, education and training within scope of practice.

The Per-Diem Visitation Specialist is responsible for scheduling and supervising visits for infants, toddlers, and children with their families at the Reception Center. This role will work closely with ACS to support established visitation plans. Overview of the Role
The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program, and the Therapeutic Group Home programs located at our licensed residential facility in Yonkers, NY as well as a Transitional Foster Care program and the Long Term Foster CareProgram located in foster homes in the Bronx, Queens and Westchester. In all programs, we provide therapeutic and educational services and other supports that youth need to achieve success through an array of strength-based and targeted interventions as well as link them with family members and/or other sponsors in the United States so they are able to move forward with their lives and thrive. The Home Finder Supervisor will be responsible for the overall functioning of the Foster Home Program by consistently meeting OCFS and ORR mandates.
